Acknowledgment of Risks

I, the undersigned, understand and acknowledge that participation in martial arts, karate, kickboxing, MMA, fitness, and related activities at All Valley Martial Arts involves physical contact, strenuous activity, and inherent risks including, but not limited to:

Falls, collisions, or strikes

Physical contact with instructors or other students

Sprains, strains, broken bones, bruises, or other injuries

In rare cases, serious injury or death

I voluntarily choose to participate (or allow my child to participate) in these activities with full knowledge of these risks.

Release of Liability

In consideration of being permitted to participate at All Valley Martial Arts, I hereby release, waive, discharge, and covenant not to sue All Valley Martial Arts, its owners, instructors, employees, volunteers, or agents from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, or injury, including death, that may occur while participating in martial arts activities, whether caused by negligence or otherwise.

Medical Authorization

In the event of an emergency, I authorize All Valley Martial Arts staff to obtain medical treatment for myself or my child, and I assume full responsibility for all medical expenses incurred as a result of such treatment.

Photography/Video Release

I grant permission for All Valley Martial Arts to use photos or videos of myself/my child taken during classes or events for promotional purposes, without compensation.

Agreement

I have read this waiver, fully understand its terms, and sign it freely and voluntarily. I acknowledge that by signing this document I am giving up substantial legal rights.
